Un mésothéliome est une tumeur maligne des surfaces mésothéliales touchant principalement la plèvre, moins souvent le péritoine et plus rarement le péricarde. Le tabac ne joue aucun rôle dans le risque de survenue d'un mésothéliome.

Cette tumeur maligne rare se développe le plus souvent sur la plèvre dans le thorax, plus rarement sur le péritoine dans l’abdomen. Le mésothéliome est trois fois plus fréquent chez les hommes que chez les femmes, à cause d’un risque lié à une exposition professionnelle qui en fait un modèle de cancer lié à l’environnement, en l’occurrence à l’amiante, ou asbeste. Cette substance est connue depuis l’Antiquité pour sa résistance à la combustion et, dès cette époque, l’asbestose provoque une insuffisance respiratoire rattachée à une activité professionnelle liée à sa manipulation (esclaves des mines d’amiante). Ce n’est cependant qu’en 1960 qu’un rapport étroit est établi entre le mésothéliome et la manipulation de l’amiante.

Ce danger affectait les personnes travaillant directement avec elle (service d’isolation thermique, industrie des freins, fabrication de plaques en fribrociment), mais aussi, en dehors des lieux de travail, les personnes vivant auprès des travailleurs de l’amiante (famille, commerçants...). Ce danger est majoré par la fumée de cigarettes qui facilite le transfert de l’asbeste au niveau des voies respiratoires. Ce sont les fibres les plus fines, particulièrement indestructibles et qui atteignent la partie la plus profonde du système bronchique, qui sont responsables du développement du mésothéliome, par irritation physique plus que chimique. Cette association asbestose-mésothéliome a motivé une législation pour protéger les personnes exposées dans les industries utilisant encore l’amiante. Le mésothéliome est reconnu comme maladie professionnelle prise en charge lorsque la relation avec l’amiante est médicalement caractérisée (collège de trois médecins). Le contrôle encore trop récent de l’exposition n’évitera pas dans les dix ans à venir l’augmentation significative du nombre de cas à travers le monde.
